Real Answers to Key Questions Quitting cigarettes is a major physical and mental challenge. To find out how to make quitting easier-and more successful-we went to leading nicotine researcher Neal L. Benowitz, M.D., chief of the Division of Clinical Pharmacology and Experimental Therapeutics at the University of California in San Francisco. Q: I've tried quitting before, but have always failed. I guess my willpower just isn't strong enough. How can I stop smoking once and for all? A: Don't be so hard on yourself! The success rate for people who try to quit cold turkey without any help is 5 percent. However, we now have a variety of cessation therapies that can increase your chances of success by lessening withdrawal symptoms such as irritability, lethargy, trouble concentrating and hunger. Here is what's available: Nicotine Replacement Therapy (NRT) comes in different forms. The gum (such as NicoretteŽ) delivers a dose of nicotine when you want it and gives you oral satisfaction. The patch (such as NicoDermŽ CQŽ and NicotrolŽ) delivers small doses of nicotine continuously for hours, and is a good choice if you want a steady level of medicine or don't like the gum. Also available by prescription are a nicotine inhaler and nasal spray. In addition, there's bupropion, an antidepressant sold under the brand name ZybanŽ, which research has found helps suppress the urge to smoke. Q: Are nicotine replacement products safe? Aren't they just as addictive as cigarettes? A: Nicotine replacement products are safe. Nicotine, when used in such products as the patch or the gum, has not been shown to cause cancer. Cigarette smoke, however, is composed of over 4,000 chemicals, including cyanide, formaldehyde, carbon monoxide, radioactive substances and carcinogens. The nicotine in cigarettes is dangerous because its rapid delivery to the brain makes you want to keep smoking. But while a cigarette gives you a huge hit of nicotine within 10 seconds of inhaling, the doses from NRT are smaller and delivered gradually, making it much less addictive and easier for you to eliminate your nicotine habit altogether. Q: Do I have to see my doctor to get a prescription? A: The nicotine gum and some nicotine patches are available over the counter. The other therapies I've mentioned require a prescription. Q: Will nicotine replacement therapy take care of my cravings for cigarettes? A: No. NRT is not a magic bullet. It will help relieve your physical discomfort, but you'll still need to deal with the psychosocial aspects of your habit. You may associate smoking with socializing, working and stress reduction. To quit, you'll need to change these ingrained behaviors as well. To help you, some NRT products come with support programs that can be personalized to meet your needs. You can also find support groups through your doctor or local hospital. They're great because you'll learn tips from those who have been there. Yes, quitting smoking is tough. But don't be discouraged. Remind yourself that quitting means staying healthy, saving money and keeping those you love from the many dangers of secondhand smoke. Good luck! Water Works You've heard it a million times: Drink eight glasses of water a day for good health. But did you know that finally taking that advice could also make quitting cigarettes a whole lot easier? The reason: "Dehydration aggravates nicotine-withdrawal symptoms," says Felicia Busch, R.D., a spokesperson for the American Dietetic Association. In fact, in order to help keep withdrawal symptoms at bay, Busch recommends drinking even a little more-10 eight-ounce glasses of water each day. If that seems like a lot, keep in mind that you can also fulfill your fluid requirement with soups, juices, even such juicy fruits as oranges, pineapples and grapes (11Ÿ2 cups of fruit equals a cup of fluid).
